- The student will receive further notice regarding the date of actual enrollment and the list of requirements to be submitted thru sms.
Requirements to be submitted
Incoming Grade 11
- Original Grade 10 Report Card (From 138)
- Original Form 137 - If the request is needed you may contact the Testing and Admission Office
- Certificate of Good Moral Character
- PSA/NSO Authenticated Birth Certificate
- 1pc. of recent 2x2 photo (White Background)
- Fully accomplished ICT-ED SHS Admission Form
- 1 Long Brown envelop to contain the above requirements
Transferee
- Original Report Card from the previous school (From 138)
- Original Transcript of Record (FORM 137) with "FOR ICT-ED INSTITUTE OF SCIENCE AND TECHNOLOGY INC." label
- Certificate of Good Moral Character
- 1pc. of recent 2x2 photo (White Background)
- Fully accomplished ICT-ED SHS Admission Form
- 1 Long Brown envelop to contain the above requirements
